is care planning to frail elderly 60+ years.
Referrals made to appropriate services to help
clients remain at home. OASIS provides in-home
client assessments and individual care plans for
the frail elderly over 60 years of age.
The
goal of
OASIS is to keep seniors living independently for
as long as possible through case management and
volunteers.
